Guardianship Reporting
As a guardian, you are required to file reports as detailed in the Superior Court Judgment of Incapacity and Appointment of Guardian(s) filed upon completion of your guardianship hearing.
If you do not file the reports on time, you will receive a reminder from the State of New Jersey Guardianship Monitoring Program of the delinquency and reminder to file. If the reminder is not sent, you are still responsible for the timely filing of all guardianship reporting forms.
Annual Reports
These reports are due every year within 14 days of the anniversary of your judgment date. Your judgment and reminder letter will tell you which of these reports to complete. Additionally, at the time of your appointment, this office provided you with a Judgment Information Sheet, that outlined reporting requirements and timelines. Please refer to that information sheet for assistance.

Again, refer to the Judgment of Incapacity and the provided Judgment Information Sheet for what reporting is required of you.
Reports require a filing fee of $5.00 per page payable to the county Surrogate. Contact our office for payment details.

Amended Report
If you need to fix a report that you have already filed, use the report on the website, and change the title to “Amended”
Final Reports
Is your ward, the incapacitated person, now deceased? You are required to file final reports so that any remaining funds can be transferred to the deceased person’s estate for administration. Add “Final” to the report title when you complete then provide to our office for filing with the fee.
Report Of Well-Being Tips & Reminder:
- It is your responsibility as legal guardian to be aware of your ward’s medical care
- Answer every question yourself, even if your ward is in a facility
- Do not refer to attachments in your answers, include that information in your report
- The Certification by Examining Professional is to be completed by any doctor who has treated your ward within the last reporting year – no other medical documents need to be included; please do not attach IHP reports. (Individualized Health Plan)
- Co-Guardians reminders
All co-guardians must report. Co-Guardians may all sign the same reports or send in their own reports. Either way, there must be a cover page for each co-guardian. (Co-Guardians who live together may complete one cover page with all names.)
Accounting Tips & Reminders
- You do not need to supply statements unless you are asked for them
- Please label your Accounting with your reporting range dates
- This year’s beginning balance should match last year’s ending balance
- There is a $5 per page fee for financial reports
